SuperMed PPO Plus
Fleet Owners offers health insurance plans to meet the needs of the Members and families of our Participating Employers. The plans utilize a network of hospitals, doctors and other health care providers. Members are able to view benefit snap shots, medical deductibles, claims and healthy living programs on the Medical Mutual website.
Members can look through the provided SuperMed Network Directory to view the list of providers or they can check out the Medicial Mutual website to view a list of providers. Look through the Summary Plan Description Booklet or the Summary Plan Shell to see what your plan covers.

Members are covered by a Tradional Dental Plan, so they can go to any dentist, but the Fleet Owners Insurance Fund offers a dental overlay through Dentemax. The Dentemax overlay helps the member keep the cost of their dental care down through a negotiated rates with providers. The dental plan is designed to cover most dental services, including preventative services, essential/restorative services, complex services, and cosmetic services. Members can look over the supplied Dental Directory or check out the Medical Mutual Website to view a list of providers. Members are able to go to any Optical location, but it is more cost effective to go to a provider within the Medical Mutual network. The vision plan options include network based plans that provide coverage for eye examinations, frames, prescription lenses and contact lenses. Through Medical Mutual Members have access to healthy living. Members should create a user name and password on the Medical Mutual website. Once logged in click on the healthy living tab to have access to member programs, coaching services and practical information. Members will have access to:

- Nutrition: make positive choices through developing healthy nutrition strategies, daily and weekly diet guidelines, and dietary do's & don'ts.

Drugstore.com: search through 45,000 over-the-counter health, beauty ans household items at everyday low prices.
CVS Caremark/ Medtipster
The prescription drug program allows members to fill prescriptions at more than 62,000 participating retail pharmacies nationwide and through Caremark Mail Service Pharmacy. To help you control the cost of prescription medications, ask your doctor to prescribe you with generic medications.

Mail Order Precscriptions
The Caremark Mail Sevice Pharmacy is a convenient option for all maintenance or long-term medicine. You are able to request up to a 90-day supply of medicine. If you are Interested in using the mail order service for your maintenance prescriptions you are able to apply through the mail by contacting us to recieve the order form, printing out the order form below or order online through the Caremark website.
Have your member ID number from your prescription card and your RX number or medication name available to register.

Visit the Medtipster website to find discounted generic prescriptions at local pharmacies. Have available the name of the drug, dosage amount and zip code.
File a Claim
Need to file a claim for disability, hospital or surgical benefits? Please contact us to recieve the required claim form or download the document below. Report the claim within 20 days to Fleet Owners and submit the fully completed form to Fleet Owners Insurance Fund within 90 days from the end of the disability.
Change in Member Status
If you need to update your address, marital staus or dependents, please fill out the document below and return it to the Fleet Owners Insurance Fund. It is important to keep the Fund up to date with any changes. Please contact us with any questions.
